An employee was placing a forklift chain back onto the roller. As he was doing this, the mast/forks dropped and his thumbs were caught between the chain and roller, resulting in the amputation of his thumbs.

An employee was moving a forklift mounting plate from a shelf onto a pallet. The pallet was lifted on a forklift and the mounting plates were slid from a 3.5-feet-tall shelf to the pallet. The plates have a notch in them, and while the employee was guiding the second plate onto the pallet, his right index finger was amputated at the first knuckle.

An employee was performing maintenance on the B Extruder when he sustained a left pinky finger first digit amputation and fracture. The employee was removing an upper roll for the flat belt. While supporting the roll with one hand and removing bolts with the other hand, the employee lost control of the roll pinching/smashing the left pinky finger between two rollers.

An employee was working in Department 24 at the "B" line boxing station. Welding rods were moving on the indexing cooling conveyor that carries them from the furnace. The employee was assisting the movement of the rods along the conveyor with his left hand when his right thumb was caught in a pinch point in the conveyor resulting in a fingertip amputation.

An employee was standing at the rear of a machine and the machine's mesh tray fell down onto the rear of the machine, crushing his left hand against the read lift. The employee sustained fractures and a laceration to their left hand and was hospitalized.

Fairfield Development, LP

An employee had been tasked with hauling large rocks. There was a rock caught in the tailgate of the dump truck that was preventing it from opening. He went to get the rock out and the tailgate came down, crushing his right middle fingertip. The employee sustained a fracture of the distal phalanx and required surgery.

An employee was replacing the wear pan in a wood chipper when a metal sawdust pipe attached to the machine fell from above, pinning him against the machine. The employee suffered a broken right femur, right hip, and left hip.

Lift Solutions Inc.

Two employees were working on a forklift with its mast in the air when a chain came loose and the carriage fell down on them. The injured employee was hospitalized with a fractured left ankle, a laceration on the left side of their forehead, and a fractured L1 vertebra.

Fucich Contracting, Inc.

An employee was hooking up a hydraulic attachment on a forklift. When pressure was released from the hydraulic system's hose, the arm of the forklift came down on the employee's left foot and broke it.

An employee was moving metal out of a cart. He had removed one piece and was working to move another when the metal shifted. His right ring finger was crushed between the two pieces, and he suffered a partial amputation to the finger pad at the tip.

Coastal Welding Supply

An employee was at the loading dock working to manually move an oversized load off of a pallet that was sitting on another pallet. The oversized load (a gas cylinder cradle) tipped over and crushed the employee's right leg against the ground. The employee sustained a fracture to their ankle/leg requiring surgery.

An employee was depressurizing a line stop actuator when their left arm was struck by a threaded, 2-inch nipple from the side of the line stop housing. The employee sustained fractures to their left arm and a rib.

D & S Welding, LLC

An employee was cutting the top of an empty scrap barrel that previously contained antifreeze. The bottom of the barrel blew out due to pressure that built up and caused his jeans to catch fire, resulting in burns.

An employee was moving extra wire out of a cleaning tank by using the tank's jogging function. Two of the employee's right-hand fingers were caught on the last strand going around the acid roll, and the employee suffered an amputation to the right little fingertip.

United States Postal Service

An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

GXO Logistics

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.
