An employee was operating a cone/spear punch press and went to remove a metal piece when the machine cycled. His thumb got caught between the cut rear section of the converter and the internal parts remaining inside and was crushed. The employee was hospitalized.

The employee was descending a 6-foot A-frame ladder when they slipped and fell to the ground from the third or fourth rung from the bottom. The employee sustained a fractured left humerus.
An employee was assisting in an inventory count by removing a pallet of goods from the second-to-top shelf of an inventory rack using a forklift. The employee's forklift struck the cross beam of the shelf above and the pallet became unstable. A few boxes fell off the pallet, striking the forklift's protective cage.
