An employee was hot patching a tire, which involves lighting a flammable liquid on fire to patch the tire. The flammable liquid contacted his arm, and he sustained burns to his arms and face.

On July 19, 2025, an employee was filling a wheelbarrow tire with air when the tire exploded, resulting in severe injuries to their right hand including a partial amputation.

An employee was placing an automotive tire on a balance and mount machine when the tire crushed their left ring fingertip, resulting in a fingertip amputation that was surgically reattached.

Two employees were attempting to align a car with an alignment rack, but the rack was not reading the car properly. The employee in the car attempted to move the car back and forth so the rack could read it when the car rolled off the rack and struck the injured employee, resulting in a fractured leg.

An employee was operating a forklift on the airport ramp environment to help offload cargo pallets from a tractor trailer. He was manually locking the cargo dolly (a specialized piece of ground support equipment used to transfer pallets) to prevent the load device from shifting or falling off the dolly during transportation. The employee stepped between the K-loader and the dolly to put up a lock when they became pinned between the loader and the dolly, resulting in inflammation to their left leg. The employee was hospitalized.

An employee had been loading a trailer. Their foot was caught in the trailer and/or trailer hitch, resulting in a severe laceration to a toe that required surgical amputation.

An employee had been trimming trees. They were found unresponsive in the cabin of the trimmer. The employee suffered a concussion and was hospitalized.

The injured employee was standing on a form/bed where concrete is poured and was removing concrete from the form(s). Another employee indicated through hand signals to the injured employee that the chocks had not been removed from the strands. The employee jumped down from the bed to ground level. The bed then started moving toward him. He went to jump over the drive wheel to get out of the way of the bed, but his foot got caught in the drive wheel. The employee sustained a severe laceration and crush injuries to the foot that required surgery.

An employee was preparing meals in a kitchen when an object struck their head, resulting in a laceration to the head and a swollen right eye. The employee was hospitalized and required surgery for a brain injury.

An employee was replacing the cutting edges on the bucket of a wheel loader. The bucket had been lowered onto composite blocking to prevent it from falling to the floor. The blocking dislodged and the bucket fell and crushed the employee's left hand.

An employee was walking on top of a piece of equipment while engaged in repair activities. The employee stepped onto a rotating impeller shaft of a belt-driven exhaust fan and sustained a partial amputation to their right leg below the knee and amputation of their left hand.

An employee was installing an automobile lift at a client location and put the lift on a safety lock. The employee's right hand was caught in the lift and crushed, causing the amputation of the little finger at the last knuckle and all of his right fingers were fractured.

On April 18, 2025, an employee was cutting the lid off an empty 55-gallon drum using a plasma cutter. The drum originally contained a flammable liquid and residual liquid inside the drum ignited and exploded. The employee was hospitalized with burns to the face, chest, and arms.

An employee was repairing a customer's heavy truck when his right index finger was crushed between the truck and a transfer case. The finger was amputated at the fingernail.

An employee tripped over a stool and fell on the floor, resulting in a left displaced mid-cervical femoral neck fracture.

An employee was installing fence posts when they stepped into a recently dug hole that was not visible due to rainfall. The employee's leg was fractured.

An employee assisted in cleaning material from a conveyor pit. After the pit was cleaned, the employee proceeded to replace metal safety plates to ensure other employees did not fall into the pit. While replacing one of the last plates, the employee mis-stepped and fell approximately 5 feet into the pit. The employee was hospitalized with back/side bruising, elbow bruising, bone bruises, and/or fractured ribs.

An employee was inspecting a breaker box and turning on/off a circuit breaker when he was electrocuted.

An employee was walking on the sidewalk. When they stepped off the curb, they fell to the ground, resulting in fractures to their tibia, fibula, and a metatarsal.
