An employee was helping to place a scale (approximately 4' x 4') on a work stand. They were aligning the scale when the chain supporting the scale was pulled, causing their left thumb to get caught between the scale and the work stand. The thumb was fractured and the tip was amputated.

The last joints of the middle and ring fingers were amputated, and the last joint of the index finger was partially crushed.

The employee was pinned between the drive shaft and two pieces of angle steel, resulting in a fracture to their left hip.
