A service repair technician was installing a heavy-duty truck flywheel when it fell. The employee went to catch it, and their right middle fingertip was caught between the flywheel and the concrete floor. The employee sustained an amputation of the fingertip.

An employee was walking past a crossover conveyor when a ramp came down and hit them in the back. The employee sustained a broken back vertebra, as well as a concussion, and was hospitalized.
