Final narrative
An employee loaded 18x42 and 24x42 steel pipe that was banded together onto the back of a flatbed trailer. The employee was securing the pipes with a strap over the top of them. When the strap tightened, the band broke and the pipe rolled off onto the employee. The employee sustained fractures to the left arm, hand and leg.

An employee was pulling stainless expanded metal from a rack. The metal lacerated the employee's right hand, cutting through an artery and a tendon. The employee was hospitalized and underwent surgery.
At 12:30 p.m. on March 11, 2021, an employee fell off a burning table while removing scrap from it. He landed on his arm on the concrete floor, suffering a fracture to the elbow/arm that required hospitalization and surgery.
An employee was banding a bundle of cut flat steel bars with an overhead crane when the bundle slipped out of the hoist chains and fell onto the employee's right middle finger, crushing and breaking it.
The injured employee was assisting five other employees with bracing a wall panel that was being stood up. Another employee slipped on frost on the floor surface. An employee was attempting to lift a steel beam from a horizontal, stacked position to a vertical position using a crane. They had placed two hooks on the east side of the beam on the flange. As the employee was hoisting the crane, the beam began to shift and pulled the employee forward onto the stack. When the material shifted, the hooks released and the material fell, pinning the employee between the two beams. The employee was hospitalized with soft tissue contusions on their proximal right thigh and interior left thigh.
An employee was moving a tensile strength test device with a dolly. The device fell from the dolly onto the employee's right leg, causing a compound fracture.
An employee used a crane to set down a bar. The bar measured 9 inches by 16 feet. The strap got stuck under the bar, and the employee asked another employee to jog the roll. The bar rolled and pinched the injured employee's left hand, resulting in a fingertip amputation.
