An employee was operating a pull cord for a batch machine when they lost balance and fell to the ground. The employee sustained fractures to their tibia and fibula.

A spreader chain was holding a tailgate open. The employee pulled the chain out while the employee's finger was between a box and the gate. The tailgate then smashed and severed the employee's right ring fingernail and index finger at the distal joint.
An employee was performing demolition activities. The employee removed a ceiling tile and liquid from a water pipe spilled onto the employee. Shortly thereafter, the employee suffered lack of muscle control, nervous system depression, and memory loss. The employee was hospitalized.
An employee was moving a 3-ton condensing unit, strapped down on a dolly, out of a garage. The strap broke, causing the employee to fall backward onto the brick pavered driveway. The employee suffered injury to a spinal ligament in the neck.
