Federal OSHA recorded a severe workplace injury at DeShazo, LLC, 10825 Telge Road, HOUSTON, TEXAS 77095 on — Fractures , affecting the Trunk and other upper extremities.

An employee was working 32 feet above the ground in a scissor lift, attaching a camera to a gantry crane. The crane moved and struck the scissor lift, which tipped and fell to the ground. The employee was hospitalized with fractures to his collarbone, humerus, and pelvis. Fall protection was worn at the time.

Deshazo, LLC.

An employee was on a scissor lift approximately 20 feet high when a forklift made contact with the aerial work platform causing it to tip over. The employee was hospitalized with a head injury.

DeShazo LLC

An employee had taken all parts and tools off a steel drop table. One half of the drop table rested partially on the other half of the drop table. When the employee closed the door near the drop table pieces, the vibration caused one piece of the drop table to slide and fall onto the employee's left foot, breaking several metatarsal bones.

An employee was picking up a metal extension for a forklift when the equipment fell on his hand and partially amputated his right middle finger.

Employees were installing components of an elevator from inside the elevator and removed temporary hoisting equipment when the elevator descended from the fourth floor to the pit floor. One employee was hospitalized with fractures to their fibula and a rib, and the other employee was hospitalized with fractures to both feet.

An employee was disassembling a pressurized unit using a driver drill. When the unit was disassembled, it caused a kickback from the tube head, causing the drill to strike the employee's left forearm. They sustained a diaphyseal fracture to their left radius and ulna, which required surgery.

After repairing a piece of industrial equipment, an employee had brake-cleaning liquid on his gloves. A lighter ignited the liquid, and the employee's hands were severely burned.
