An employee was rearranging parts inside a rack when he fell from a dock area to the concrete parking lot 4 feet below. He suffered a broken left humerus.

The injured employee used a step ladder to climb on top of a mold. Another employee was on the step ladder assisting him and handing him tools as needed. The injured employee was startled and fell approximately 7 feet from the mold to the concrete floor, sustaining head and back injuries.

An employee was descending a 10-foot A-frame ladder. The ladder came out from under the employee when he was about 5 to 6 feet above ground level. He fell, landed on the left side of his body, and sustained fractures to four ribs, his pelvis, and hip socket, and sustained injuries on the left side of his body. The employee was hospitalized.

An employee was handling a small platform ladder with three steps and two wheels in the front. The employee was found on the floor underneath the ladder. She was hospitalized with multiple fractures to the sacrum.

An employee was on a ladder four feet above the ground to install insulation. The ladder began to slide and the employee jumped to the shop floor, breaking his left tibia and fibula at the ankle. He was hospitalized, requiring surgery.

On July 21, 2025, an employee was working from a 10-foot step ladder while installing a 3-by-5-foot glass window pane on the outside of a new home under construction. The window fell from the frame and the employee went to catch it when the ladder slipped. The employee fell 5 feet to the ground and landed on small pieces of concrete left over from the foundation pour. The employee was hospitalized with a dislocated elbow and fractured wrist that required surgery.

An employee was operating a drill and tap machine. He was placing a part with his right hand and removing a part coming in on the left side. He saw a part that was crooked and went to straighten it. The machine clamped down on his left index finger, degloving the top part of the finger and resulting in a partial amputation.

An employee was performing maintenance on a roll cutting machine in a warehouse. The employee was removing excess grease from the bearings when the machine's chain caught their right index finger. The fingertip was amputated by the chain and sprocket.

During fiberglass work, an employee mixed a methyl ethyl ketone peroxide (MEKP) catalyst with a cobalt chemical that they thought was a dye. A chemical reaction occurred, resulting in an explosion. The employee sustained second-degree burns on his back and a blister on his right arm.

An employee was walking when she tripped over a box on the ground and fell. The employee was hospitalized with a severe dislocation to the hip and leg that required surgery.
