An employee's telescopic forklift stopped functioning at an intersection of a temporary roadway. The employee leaned over to locate the fuse compartment. Another telescopic forklift backed into the disabled forklift at the intersection. The employee was pinned between both machines. The employee was hospitalized with a fractured lumbar vertebra.

RAILROAD CONSTRUCTION CO., INC.

Employees were on a scissor lift, engaged in ceiling repairs within the center tube of the tunnel. A dump truck struck the scissor lift, causing it to topple over. The injured employee was inside the scissor lift when it fell. The employee sustained multiple injuries, including fractures, a head contusion, bulging discs, and lacerations to multiple body parts. The employee was hospitalized.

An employee was inside a truck bucket, positioning it to perform maintenance on an overhead sign. A semi-truck in an active lane near the work zone struck the bucket the employee was in, causing the bucket to break and the employee to be ejected. His fall protection harness caught him before he hit the ground. He sustained a fractured left tibia.

An employee was parked along the side of a road working on a fiber outage. An automobile driven by an intoxicated person crossed over into the designated work zone and struck the employee's truck. The employee sustained injuries to his chest, lungs, shoulder, and head.
