29 CFR 1910.23(d)(1):   Every flight of stairs having four or more risers was not equipped with standard stair railings or standard handrails as specified in paragraphs (d)(1)(i) through (v) of this section, the width of the stair to be measured clear of all obstructions except handrails:    Employees working on drilling rig #3 at SEC 16, TWP 35, R 11, 610 fsl 315 fel, Kiowa, KS 67070 were exposed to slip and trip hazards in that, the flight of stairs consisting of five risers leading to the pipe rack was not equipped with standard stair railings or standard handrails.

29 CFR 1910.23(c)(1):   Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings:      Employees working on drilling rig #3 at SEC 16, TWP 35, R 11, 610 fsl 315 fel, Kiowa, KS 67070 were exposed to fall hazards of approximately 6' from the catwalk area around the mud tanks to the ground below in that sections of guardrails were not installed.    Fossil Drilling, Inc was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.23(e)(1), which was contained in OSHA inspection number 311787790, citation number 2, item number 1 and was affirmed as a final order on 5/2/2008, with respect to a workplace located at SEC 22, TWP 25, R 13, Byers, KS 67021.

29 CFR 1910.212(a)(1):   Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by the high pressure mud pump drive rods:    Employees working on drilling rig #3 at SEC 16, TWP 35, R 11, 610 fsl 315 fel, Kiowa, KS 67070 were exposed to caught-by hazards in that, the "pony" rods for the high pressure mud pumps were not guarded.    Fossil Drilling, Inc was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 311787790, citation number 1, item number 3 and was affirmed as a final order on 5/2/2008, with respect to a workplace located at SEC 22, TWP 25, R 13, Byers, KS 67021.
