29 CFR 1910.132(d)(1)(i): The employer did not select and have each affected employee use, the types of personal protective equipment that would protect the affected employee(s) from the hazards identified in the hazard assessment: Employees performing well logging tasks at SEC 3, TWP 33, R 5, 1465 fnl, 2475 fwl, Winfield, Kansas were exposed to fire and explosion hazards. Employees had reached total bottom depth performing well logging at or near the expected hydrocarbon zone and were not wearing Flame Resistant clothing.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use: Employees performing mud mixing tasks on Rig #? located at SEC 3, TWP 33, R 5, 1465 fnl, 2475 fwl, Winfield, Kansas were exposed to eye injuries and chemical burns. The eye wash located near the mixing skid was missing the lid and only had 2 inches of water in the bottom.
