Transformers Construction services, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 315481929, Serious citation number 01, item number 002 and was affirmed as a final order on February 1, 2012 with respect to a workplace located at 12298 Water Tupelo St. Jacksonville, Florida. 29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system: a. On or about October 17, 2013, on the roof trusses of the 5:12 pitch roof, employees installing framing were exposed to a 20 feet exterior fall hazard in that they were not protected by a fall protection system. b. On or about October 17, 2013, in the roof trusses, an employee installing framing was exposed to a 13 feet fall hazard in that he was not protected by a fall protection system. c. On or about October 17, 2013, on the second floor, employees installing framing near the stairwell opening were exposed to a 10 feet fall hazard in that they were not protected by a fall protection system.
