OSH ACT of 1970 Section (5)(a)(1): Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall and crushing hazards:  a.   On or about October 1, 2014, in the roof trusses, four employees installing hip roof framing and interior wall framing were exposed to 8 to 16 feet fall hazards, crush hazards, and struck-by hazards from a potential structural collapse in that the employer did not adequately brace roof trusses by follow the temporary bracing plan specified in the design engineer's Truss Design Drawings that referenced the Building Component Safety Information (BCSI) Guide to Good Practice for Handling, Installing, Restraining & Bracing of Metal Plate Connected Wood Trusses, the employer's source of recognized and generally accepted good engineering practices for temporary bracing of wood truss structures.

29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, a personal fall arrest system, or an alternative fall protection measure as provided by another provision in paragraph (b) of this section:  a.   Employees performing residential framing work at elevations of six feet or more above the ground are not protected from fall hazards by a fall protection system.  The employer fails to assure effective protection against falls as required by the standard, as demonstrated on October 1, 2014, when employees were installing framing from the first and second story roof trusses of a residential house at 153 Forest Edge Drive, Saint Johns, Florida 32259 with out fall protections and were exposed to fall hazards of eight, twelve, and sixteen feet above lower levels.  Transformers Construction Services, Inc. previously committed a Willful violation of this standard with an 11(b) warning on April 10, 2014, when employees were working on a residential house at 62 Stirlingshire Court, Saint Augustine, Florida, 32259, which became a Final Order on August 6, 2014. (Inspection No. 967068)  Transformers Construction Services, Inc. previously committed a Repeat violation of this standard on January 17, 2014, when employees were working on a residential house at 106 Cantley Way, Saint Augustine, Florida, 32259, which became a Final Order on April 2, 2014. (Inspection No. 955478)  Transformers Construction Services, Inc. previously committed a Repeat violation of this standard on January 15, 2014, when employees were working on a residential house at 1274 Camp Ridge Lane, Middleburg, Florida, 32068, which became a Final Order on May 5, 2014.
