29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition.    a)    Employee was cleaning up at ground level around the butt grinder conveyor with a log butt approximately 14' above her resting partially on the conveyor housing which could have fallen off on top of her.    b)    In the Electrical room by the wood chipper had wood chips up to 8" deep in the electrical room in some areas.    c)    The Chipper chain drive area had saw dust up to 3" deep on pipes in the area outside of electrical room.    d)    In the Small bay in the new fuel house had bricks on the ground and being knocked out of the wall surrounding the opening.    e)   The Boiler room doors were damaged and falling off with broken windows.    f)    Abandoned storage building where parts were stored had doors falling off exposed employees to struck-by injuires.    g)    Wood chips covered the stairs leading up to the debarking deck.    h)    The Green chain room had damaged light fixture and wood hanging from the ceiling.

29 CFR 1910.213(a)(9): All belts, pulleys, gears, shafts, and moving parts were not guarded in accordance with the specific requires of 1910.219:    a)    The Rosser head waste conveyor guard was damaged and did not protect ingoing nip for chain and sprocket in the yard.    b)    The Craftsman 12" band saw did not have a guard on the belt and pulley drive.    c)    The Rosser conveyor tail pulley section had an unguarded chain and sprocket drive.    d)    The peeling de barker tree drive shaft was exposed where the guard was out of position.    e)    The de barker in the addition did not have belts and pulleys guarded or area protected during operation.     f)    The shaker conveyor ingoing nip point was not guarded at platform level.    g)    The core conveyor outside of building had an unguarded 2" diameter shaft sticking out 5".    h)    The chipper needed additional guarding for the chain and sprocket drive.    I)    The conveyor feeding the chipper had an unguarded 1 1/2" shaft that was sticking out 2 1/2".    j)    The conveyor belt feeding the chipper had an unguarded ingoing nip point approximately 3" from the frame.    k)    The new core conveyor had an unguarded chain and sprocket at floor level.    l)     In the saw mill, shredder conveyor tail pulley guard was damaged and out of place.    m)    The overspill conveyor for the shredder belt needs to be guarded which is approximately 6' 2" off the floor to nip point.    n)    The 2nd barn cleaner from the de barker tail pulley needs guarding approximately 43" off the floor.    o)    The short side 2nd saw dust belt and pulley need guarding 82" off the floor.    P)    The chipper injector chain and sprocket drive unit needs to be fully guarded.
