29 CFR 1910.22(a)(3): Walking-working surfaces were not maintained free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, spills, snow, and ice:  a)    On or about 7/19/2017, floors in the basement of the mill contained slip trip and fall hazards:          1.    West wall near boiler room had approximately a 8" wide x 10" long hole in the floor approximately 4" deep.          2.    Near electrical panel "H" there was a depression in the floor approximately 3' x 4' x 2" deep.          3.    East wall driveway has uneven pavement approximately 2' x 3' x 3" different.
