29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): (a) On or about August 19, 2015 in the Green End; employees who performed servicing and maintenance when a reel with veneer jammed on the carriage (no manufacturer or serial number) were not required to: 1) know the type and magnitude of the energy, the hazards of the energy to be controlled, and the method or means to control the energy, 2) turn off and shut down the machine, 3) physically locate and operate energy isolating devices in such a manner as to isolate the machine or equipment from the energy source, 4) affix lockout or tagout devices to each energy isolating device. Employees were exposed to struck by and crushing hazards from the energized carriage when they climbed/stepped on the rails in order to get to a floor level close to the stuck reel and movements from the carriage arm which went up and down.

29 CFR 1910.23(c)(3): Regardless of height, open-sided floors, walkways, platforms, or runways above or adjacent to dangerous equipment, pickling or galvanizing tanks, degreasing units, and similar hazards were not guarded with a standard railing and toe board. (a) By the Debarking Coop the bottom of the stairs, a standard railing and toe board was not installed on the platform where employees would walk to reset the breakers and walk to another section of the building exposing the employees to struck by hazards. This is located on the back side of the debarker and where the 10 foot lathe had been installed. Besse Forest Products, Inc. dba Goodman Veneer & Lumber was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1910.23(c)(3), which was contained in OSHA inspection number 985617, citation number 1, item number 4 and was affirmed as a final order on January 23, 2015, with respect to a workplace located at 200 C Avenue, Goodman, Wisconsin. (b) In the Green End by the Core Conveyor Area, the platform used to cross the cores conveyor did not have a mid rail installed exposing employees to struck by hazards. Besse Forest Products, Inc. dba Goodman Veneer & Lumber was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1910.23(c)(1), which was contained in OSHA inspection number 985617, citation number 1, item number 3 and was affirmed as a final order on January 23, 2015, with respect to a workplace located at 200 C Avenue, Goodman, Wisconsin.

29 CFR 1910.213(h)(1): The sides of the lower exposed portion of the blade of radial saw(s) were not guarded to the full diameter of the blade by a device that automatically adjusted itself to the thickness of the stock and remained in contact with the material being cut: (a) In August 2015 in the Maintenance Department, the DeWalt radial arm saw 780 12" Powershop was only equipped with a lower guard on the left side of the blade while facing the saw. No guard was provided for the right side.
