29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):     (a)  On or about August 19, 2015 in the Green End; employees who performed servicing and maintenance when a reel with veneer jammed on the carriage (no manufacturer or serial number) were not required to:      1)  know the type and magnitude of the energy, the hazards of the energy to be controlled, and the method or means to control the energy,      2)  turn off and shut down the machine,      3)  physically locate and operate energy isolating devices in such a manner as to isolate the machine or equipment from the energy source,      4)  affix lockout or tagout devices to each energy isolating device.    Employees were exposed to struck by and crushing hazards from the energized carriage when they climbed/stepped on the rails in order to get to a floor level close to the stuck reel and movements from the carriage arm which went up and down.

(b)  In the Green End by the Core Conveyor Area, the platform used to cross the cores conveyor did not have a mid rail installed exposing employees to struck by hazards.
