An opening approximately 24" wide and 74" high was only protected by a piece of square tubing which was laying at a diagonal across the opening which was approximately 100" above the ground.  b)    On or about 7/5/2018, A fall hazard existed due to a missing panel in the east wall which was approximately 26" high x 20" wide.  c)    On or about 7/5/2018, A fall hazard existed on the south  end of the platform where there was an opening approximately 34" high x 54" wide with a drop of approximately 9'.  d)    On or about 7/5/2018, A fall hazard existed where the infeed log deck had bad railings with a drop of approximately 11'.  e)    On or about 7/5/2018, A fall hazard existed where grating was missing outside of the cab for an area approximately 23" x 52" with a 79" drop.
