29 CFR 1910.265(c)(4)(iv): Elevated walk(s) and runway(s), more than 6 feet or more from floor level, were not provided with both a standard railing and a toe board:  a)    On or about 7/5/2018, Fall hazards existed on a conveyor access platform near the hour glass roller. An opening approximately 24" wide and 74" high was only protected by a piece of square tubing which was laying at a diagonal across the opening which was approximately 100" above the ground.  b)    On or about 7/5/2018, A fall hazard existed due to a missing panel in the east wall which was approximately 26" high x 20" wide.  c)    On or about 7/5/2018, A fall hazard existed on the south  end of the platform where there was an opening approximately 34" high x 54" wide with a drop of approximately 9'.  d)    On or about 7/5/2018, A fall hazard existed where the infeed log deck had bad railings with a drop of approximately 11'.  e)    On or about 7/5/2018, A fall hazard existed where grating was missing outside of the cab for an area approximately 23" x 52" with a 79" drop.

29 CFR 1910.22(a)(1): The employer failed to provide a clean and sanitary condition for all places of employment, passageways, storerooms, service rooms, and walking-working surfaces.    a)    On or about 7/5/2018, Slip, trip and fall hazards existed where wood chips approximately 16" deep covered the bottom two steps for the stairs leading up to the debarking deck.    b)    On or about 7/5/2018, Walk way at bottom of bark transfer barn cleaner was covered with wood chips.    d)    On or about 7/5/2018, Walk way beside infeed log deck had extra grating and other metal parts blocking access to it.      The Besse Forest Products, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.22(a)(1), which was contained in OSHA inspection number 985617, citation number 01, item number 001 and was affirmed as a final order on January 23, 2015, with respect to a workplace located at 200 C Avenue, Goodman, WI 54125.

29 CFR 1910.213(a)(9): All belts, pulleys, gears, shafts, and moving parts were not guarded in accordance with the specific requirements of 1910.219:    a)    On or about 7/5/2018, Take away conveyor under the de barker had an ingoing nip hazard created by the unguarded chain and sprocket drive.    c)    On or about 7/5/2018, Bark transfer barn cleaner belt and pulley drive unit was not guarded creating an ingoing nip hazard.      The Besse Forest Products, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.213(a)(9), which was contained in OSHA inspection number 985617, citation number 01, item number 10 and was affirmed as a final order on January 23,2015, with respect to a workplace located at 200 C Avenue. Goodman, WI 54125.
