(b)  The site process hazard analysis (PHA) that was conducted in 2009 and re-validated in 2014 did not address facility siting in that the PHA question on whether workers in the control room or escape routes from the control room were protected from flammable sprays, fumes, mists or vapors from a process containing greater than 15,000 pounds of flammable liquids and gases was not addressed by the team as determined on October 21, 2015.    (c)  The site process hazard analysis (PHA) that was conducted in 2009 and re-validated in 2014 did not address facility siting in that the PHA question on whether workers in the control room or escape routes from the control room were protected from thermal radiation from fires, including the flare, in a process containing greater than 15,000 pounds of flammable liquids and gases was not addressed by the team as determined on October 21, 2015.    (d)  The site process hazard analysis (PHA) that was conducted in 2009 and re-validated in 2014 did not address facility siting in that the PHA question on whether workers in the control room or escape routes from the control room were protected from overpressure and projectiles from explosions from a process containing greater than 15,000 pounds of flammable liquids and gases was not addressed by the team as determined on October 21, 2015.    (e)  The site process hazard analysis (PHA) that was conducted in 2009 and re-validated in 2014 did not address facility siting in that the PHA question on whether vessels containing highly hazardous chemicals located in a process containing greater than 15,000 pounds of flammable liquids and gases are located sufficiently far from control rooms was not addressed by the team as determined on October 21, 2015.    (f)  The site process hazard analysis (PHA) that was conducted in 2009 and re-validated in 2014 did not address facility siting in that the PHA question on whether workers in the unit are protected from the effects of overpressure from explosions from adjacent facilities, such as the Dominion Gas Compressor Facility located adjacent to the Extraction Plant, was not addressed by the team as determined on October 21, 2015.    ABATEMENT CERTIFICATION REQUIRED
