Employees were working on and from Alum-A-Pole pump jack scaffold with no fall protection system in place in that end rails and netting were not installed along all open side and ends of walking/working platform exposing them to fall hazards.       b) 657 Chamberlain Avenue, Perth Amboy, NJ 08861. Employees were working on and from two job made scaffolds in that they placed 2 inch by 10 inch timber planks perpendicular to the Alum-A-Scaff platform, one spanning to a Werner A-frame ladder on the roof and the second to a Werner aluminum portable ladder. Neither scaffold platform had a guardrail system in place exposing the employees to fall hazards.       Condition occurred on or about 12/15/15.
