29 CFR 1926.102(a)(1): Eye and face protective equipment was not used when machines or operations presented potential eye or face injury:  a) 657 Chamberlain Avenue, Perth Amboy, NJ 08861, Building 3, South Side. Employee was exposed to eye and face injuries when operating a pneumatic nail gun installing new roofing materials while not wearing any eye and or face protection.     Condition occurred on or about 02/17/16.

29 CFR 1926.501(b)(1): Employee on a walking/working surface having an unprotected side or edge which is six feet or more above a lower level were not protected from falling by the use of a guardrail system, a safety net system, or personal fall arrest system:    a) 657 Chamberlain Avenue, Perth Amboy, NJ 08861, Building 3, West side. Two employees were exposed to fall hazards approximately 12 foot 6 inches from the deck surface to the ground below when installing new plywood subfloor on deck with no fall protection system in place.     Condition occurred on or about 02/17/16.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above the lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, nor was an alternative fall protection plan which meets the requirements of 29 CFR 1926.502(k) developed and implemented:      a) 657 Chamberlain Avenue, Perth Amboy, NJ 08861, Building, South Side. Employee was exposed to fall hazards approximately 12 foot from the eave to the ground below when installing new roofing materials with no fall protection system in place.       Condition occurred on or about 02/17/16.
