Essentra Plastics, LLC dba Essentra Components, 2614 McClelland Avenue, Erie, PA  16510, forklift charging area, on or about August 24, 2016 and time prior thereto:  Employees are provided with an emergency eye wash and shower when performing work operations including, but not limited to, checking forklift battery levels and adding water.  The eye wash station and emergency shower was last inspected in April 2013.

Essentra Plastics, LLC dba Essentra Components, 2614 McClelland Avenue, Erie, PA  16510, vinyl dip machine #1, on or about August 24, 2016 and time prior thereto:  A Power Sentry relocatable power tap was utilized to power electrical equipment including, but not limited to, a Dayton 9MRP6 fan and a Dayton 1UM40 fan.  The power tap was not listed and labeled for industrial use.
