Essentra Plastics, LLC dba Essentra Components, 2614 McClelland Avenue, Erie, PA: On or about October 22, 2021 and times prior thereto, employees performed servicing and maintenance on equipment including but not limited to the Dip Mold Lines. Lockout devices were not attached to Lockout points (i.e. Energy Isolating Devices) following shutdown of equipment and as employees entered the machine area to perform clean-up activities.
