B.)  Essentra Plastics, LLC dba Essentra Components, 3123 Station Road, Erie, PA  16510, tool room at mold hoist area, on or about January 12, 2017 and times prior thereto:  Storm water was leaking into the building from a leaking roof posing a slip/fall hazard.

Essentra Plastics, LLC dba Essentra Components, 3123 Station Road, Erie, PA  16510, production areas, on or about January 12, 2017 and times prior thereto:  Items including, but not limited to, adhesive floor tape and floor signage, were worn and loosing adhesion to the floor surface causing a slip/trip and fall hazard.
