29 CFR 1926.501(b)(13):  Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were other provisions of paragraph (b) of this section used to provide an alternative fall protection measure:  a) Jobsite, F1 Lot 2, Stevensan Drive, Sewickley, PA 15143:  Observed on or about January 17, 2019, employees engaged in framing activities on a two story residential structure were not protected from falling by guardrail systems, safety net system, or personal fall arrest system, nor were other provisions of paragraph (b) of this section used to provide an alternative fall protection measure.  Employees installing wooden trusses on top of the house were exposed to a fall hazard of approximately 19 feet to the ground below.       b) Jobsite, F1 Lot 2, Stevensan Drive, Sewickley, PA 15143: Observed on or about January 17, 2019, each employee walking/working on, at or above wall openings where the outside bottom edge of the wall opening was greater than 6 feet or more above lower levels were not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system.  The inside bottom edge of the wall openings on the second floor was less than 39 inches (1.0 m) above the walking/working surface.  Employees working on the second floor of the house were exposed to a fall hazard of approximately 13 feet to the ground below.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  a)  Jobsite, F1 Lot 2, Stevensan Drive, Sewickley, PA 15143:  Observed on or about January 17, 2019, employees used a Werner brand portable extension ladder to access the upper level of a two story house.  The ladder side rails did not extend at least 3 feet above the upper landing surface.  Employees using the portable ladder to access the second level of the building through a wall opening were exposed to a fall hazard of approximately 13 feet to the ground below.

The boom truck crane was being used to lift wooden trusses onto a two story residential dwelling.
