29 CFR  1926.602(d):Powered industrial truck operator was not trained in accordance with 29 CFR 1910.178(l):  Reference Standard:  1910.178(l)(2)(ii) Training shall consist of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace.  On or about September 8, 2022, times prior to and thereafter employees of Top Flight Steel Inc. were exposed to a struck by hazard working in proximity to a JLG Skytrak Model 8042 operated by an employee who had not completed practical training.
