29 CFR  1910.147(c)(1):The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  Location: Facility  On or about March 20, 2023, the employees performed maintenance operations on equipment with no program or procedures to ensure employees were protected from the release of hazardous energy while cleaning and maintaining the following equipment:  1. Hollymatic Grinder 2. Fatosa Bowl Chopper 3. Hallmark Tilt Bowl Mixer 4. Champion Tilt Bowl Mixer 5. Anko Forming and Filling Machine 6. Shaker Table 7. Weighing and Packaging Machine

29 CFR  1910.219(f)(1): Gear(s) were not guarded by a complete enclosure or by one of the methods specified in 29 CFR 1910.219(f)(1)(ii) and (f)(1)(iii):  Location: Processing Room  On or about March 13, 2023, the gears of the dumpling machine in the corner of the processing room furthest from the mixing and grinding room was found to have no guard in place with two employees working in close proximity to the unguarded gears of the forming mold set.

29 CFR  1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  Location: Processing Room and Packaging Room  On or about March 20, 2023, the filling hopper and dough hopper of four ANKO HLT-700 Multipurpose filling and forming machines as well as the forming mold of one ANKO HLT-700 Multipurpose filling and forming machines that were in use, were not guarded.  Chinese Spaghetti Factory, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 1439101, citation number 1, item number 1 and was affirmed as a final order on 12/4/2019, with respect to a workplace located at 83 Newmarket Square, Boston, MA 02118.

29 CFR  1910.303(g)(2)(i):Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating  at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i):  Location: Processing Room  On or about March 20, 2023, the back cabinet to ANKO HLT-700 multipurpose forming and filling machine was not closed and exposed employees to potential electrical shock from electronic components housed inside the machine.  Chinese Spaghetti Factory, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR  1910.303(g)(2)(i), which was contained in OSHA inspection number 1594489, citation number 3, item number 1 and was affirmed as a final order on 09/26/2022, with respect to a workplace located at 83 Newmarket Square, Boston, MA 02118
