OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause serious physical harm to employees, in that a supervisor was exposed to hazards at front and back side of the Prime Coater Machine on September 5, 2023, when he cleaned rotating chrome rollers with his hand on the front side of the machine and when he knowingly opened and entered through an awareness barrier gate with the coil coating line operating.  a) On or about September 5, 2023, at 125 McFann Rd, Valencia, PA 16059, a supervisory employee cleaned front side chrome rollers on the Prime Coater Machine while the rollers were turning with his hand and without utilizing an appropriate tool, designed to eliminate exposure to ingoing nip points. The same supervisory employee then accessed the rear of the Prime Coater Machine by opening an awareness barrier gate while the coil coating line was operating, violating safe work procedures, training, and general machine awareness safety training, resulting in fatal injuries to the supervisor.?

Employees were exposed to nip points created by the rotating rollers when they worked, walked or stood in proximity to the running rollers.  (b) Facility, 125 McFann Road, Valencia, PA, back of Prime Coater Machine: The employer does not provide one or more methods of machine guarding to protect operators and employees in the machine areas from hazards.  One instance of this violation occurred on or about September 5, 2023, when an employee performing the job of entering the area at the back of the Prime Coater and bending over to clean a chrome roller to remove a "dent" was exposed to being cut by the edge of the coil metal as it was exiting the top of the prime coater machine.  The coil of metal was in production and was running.  (c) Facility, 125 McFann Road, Valencia, PA, back of Finish Coater Machine: The employer does not provide one or more methods of machine guarding to protect operators and employees in the machine areas from hazards.  One instance of this violation occurred on or about January 22, 2024, when an employee performing the job of placing a small piece of heat tape on the underside of the coil of metal was exposed to being cut by the edge of the coil of metal as it was exiting the Finish Coater Machine.
