29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. At the construction site located in the 700 block of E. Grand Avenue, in Haysville, KS, employee(s) engaged in framing work of a new commercial structure were exposed to fall hazards in that employee(s) had fall protection on site while on walking working surfaces approximately 11 feet in height but were not using it. Employee(s) were also exposed to fall hazards in that employee(s) had fall protection on site while working on and near unprotected sides and edges which were approximately 9-10 feet in height but were not wearing it.

Grand Avenue, in Haysville, KS, employee(s) engaged in framing work on a new commercial structure were exposed to fall hazards in that employee(s) were utilizing 6-8 foot orange black A framed ladders to access an upper landing surface and the ladders did not extended at least 3 feet above the upper landing surface being accessed.
