29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a) On or about July 1, 2024, at a jobsite located at 3750 Green Meadows Dr, OAK CREEK, WI 53154 , employees conducting roofing activities to include tear-off and deck preparation on a 6:12 pitch roof of the structure were not protected from falling to the lower level. This exposed employees to a fall hazard of approximately 18 feet 6 inches on the South side of the structure at the eave and approximately 25 feet at the ridge of the roof.

29 CFR  1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  (a) On or about May 1, 2024, at a jobsite located at 523 71st St., KENOSHA, WI 53142, the employer did not insure that employees working from the roof of the structure on this jobsite had been  trained on the recognition of fall hazards and proper implementation of fall protection systems.
