29 CFR  1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  (a)  On or about December 6, 2024,  at a jobsite located at 9440 94th St, Franklin, WI 53132, employees working on the ground performing cleanup and assisting the roofers who were working directly above re-roofing the structure, were not protected from struck by hazards from falling materials and tools by wearing a protective helmet.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  (a) On or about December 6, 2024, at a jobsite located at 9440 94th St, Franklin, WI 53132, the portable ladder(s) being used by employees to access the roof of the structure  did not extend at least three feet above the upper point of contact with the eave, nor were ladders secured in place. This exposed employees accessing and egressing the roof, to a fall hazard onto grass, building materials, and  concrete.

29 CFR  1926.102(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  (a) On or about December 6, 2024, at a jobsite located at 9440 94th St, Franklin, WI 53132, employees were operating pneumatic nail guns and a leaf blower without eye protection while installing a roof. This exposed employees to struck-by and eye impalement hazards.  Trexx, LLC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR  1926.102(a)(1), which was contained in OSHA inspection number 1770478, citation number 1,  item number 1, and was affirmed as a final order on 10/15/2024, with respect to a workplace located at 940 E. Elm St. Oak Creek, WI 53154.

This exposed employees to a fall hazard from the first floor roof of approximately nine (9) feet.
