(a) Citation 1, Item 1 a serious violation of 29 CFR 1926.300(b)(2) with an abatement date of October 7, 2025.  (b) Citation 2, Item 1(a) a serious violation of 29 CFR 1926.501(b)(13) with an abatement date of October 7, 2025.  (c) Citation 2, Item 1(b) a serious violation of 29 CFR 1926.503(a)(1) with an abatement date of October 7, 2025.  (d) Citation 2, Item 2 a serious violation of 29 CFR 1926.1053(b)(1) with an abatement date of October 7, 2025. 29 CFR 1926.300(b)(2):  Belts, gears, shafts, pulleys, sprockets, spindles, drums, fly wheels, chains, or other reciprocating, rotating or moving parts of equipment shall be guarded if such parts are exposed to contact by employees or otherwise create a hazard. Guarding shall meet the requirements as set forth in American National Standards Institute, B15.1-1953 (R1958), Safety Code for Mechanical Power-Transmission Apparatus.  (a) On or about August 5, 2025, at a jobsite located at 2500 Jacob Court, Waukesha, WI 53188, employee(s) used an extension ladder hoist to convey materials up to the roof. The hoist did not have the power transmission pulley and belt guarded, exposing employees to a caught-in hazard and amputation hazard.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a) On or about August 5, 2025, at a jobsite located at 2500 Jacob Court, Waukesha, WI 53188, employees were installing rain and ice shielding on the north side of the second story of a residential roof with a 4:12 pitch, were not using fall protection. This exposed employees to a fall hazard of approximately 18 feet from roof to the ground below.  (b) On or about August 5, 2025, at a jobsite located at 2500 Jacob Court, Waukesha, WI 53188, an employee was retrieving bundles of shingles on the south side of the second story of a residential roof with a 4:12 pitch, was not using fall protection. This exposed the employee to a fall hazard of approximately 18 feet from roof to the ground below.  (c) On or about August 5, 2025, at a jobsite located at 2500 Jacob Court, Waukesha, WI 53188, an employee was performing tear off work on the north side of the first story of a residential roof with a 6:12 pitch, was not using fall protection. This exposed employees to a fall hazard of approximately 10 feet from roof to the ground below.  (d) On or about August 5, 2025, at a jobsite located at 2500 Jacob Court, Waukesha, WI 53188, an employee was performing tear off work on the north side of the first story overhang of a residential roof with a 2:12 pitch, was not using fall protection. This exposed employees to a fall hazard of approximately 10 feet from roof to the ground below.  Trexx, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1791976, citation number 2, item number 2-a, and was affirmed as a final order on May 13, 2025, with respect to a workplace located at 9440 94th St, Franklin, WI 53132.  Trexx, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1770478, citation number 1, item number 2, and was affirmed as a final order on October 15, 2024, with respect to a workplace located at 940 E. Elm St. Oak Creek, WI 53154.  Trexx, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1759024, citation number 1, item number 1, and was affirmed as a final order on October 31, 2024, with respect to a workplace located at 3750 Green Meadows Dr, Oak Creek, WI 53154. 29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  (a) On or about August 5, 2025, at a jobsite located at 2500 Jacob Court, Waukesha, WI 53188 , the portable ladder(s) being used by employees to access the roof of the structure  did not extend at least three feet above the upper point of contact with the eave, nor were ladders secured in place. This exposed employees accessing and egressing the roof, to a fall hazard onto grass, building materials, and concrete.
