An employee was walking out of the guard shack to get numbers from trailers when they slipped/tripped and fell off a curb to the pavement below. The employee was hospitalized with a hip injury.

An employee was standing on a raised platform at the seafood counter while setting the service case with product. She misstepped and fell from the platform to the floor. The employee was hospitalized with a fractured left leg and required surgery.

An employee was working in the meat department when he slipped and fell to the cement floor. He was hospitalized with a hip fracture that required surgery.

An employee was descending a fixed ladder on the side of a container loader. The employee fell from the ladder, landing on the ground about 6-8 feet below and sustained fractures to their back and pelvis.

An employee was performing maintenance/electrical work inside a substation, and used a ladder to access the top of a panel box. The employee fell 8-9 feet from the top of the panel box to the ground. The employee was hospitalized with fractures to their ribs and a shoulder injury.

An employee was driving a forklift. He fell off the machine during a turn, and one of its wheels broke his left foot. He was hospitalized.

An employee was standing at the register and tripped over a power cord that was hanging down from a shelf. He fell on his ankle and hip. The employee was hospitalized with a fractured ankle.

An employee was standing by the drink station, filling drinks. The employee turned away from the drink station, slipped on a bit of water, and fell to the floor. The employee's hip was fractured.

An employee was watching the landing joint being lowered into the rotary table when it clipped the slip bowl, causing the slip bowl to flip over and strike the employee's legs. The employee's right femur was fractured and they sustained a fracture to the left leg below the knee. The employee was hospitalized.

An employee was working from an extension ladder to assist with fabricating concrete formwork. They felt the ladder start to shift, and jumped from the ladder to the concrete ground, fracturing both ankles.

An employee was conducting pressure washing on the roof of a commercial building. The employee fell through a fiberglass skylight panel and struck the concrete pavement approximately 30 feet below. The employee sustained fractures to their pelvis, femur, and heel. The employee was using fall protection at the time of the incident, including a harness and lanyard, but it failed to arrest the fall.

An employee was cutting off bracing from an 8 x 12' metal wall when the wall fell, striking the employee on their side and back. The employee sustained bruised ribs and two fractures in their back.

An employee had been laying on a couch in the break room. While getting up, she tripped and fell to the floor. The employee was hospitalized with an injury to the left side of her head.

The injured employee was cleaning up tools and equipment around a transformer that was being assembled. Other employees were removing the mobile work platforms from around the connections lattice of the transformer. The connections lattice fell over on top of the injured employee, causing fractures to their ribs, pelvis, and foot.

An employee was walking to check the lock on the front door after closing. He slipped on the wet floor and fell. The employee's hip was fractured and required surgery.

Employees were working on a floor layout for future pipe installation. Plumbers were cleaning up the area. A piece of plywood was removed and an employee fell through a hole underneath it. He sustained fractures to the front of his head, a right side rib, and his left wrist. He also sustained lung contusions.

An employee was part of a crew that was moving a spool of wire that was on a support stand with wheels. The employee was at the back, holding the spool back while the crew moved it down the off-ramp of a buck hoist. When the support stand moved from the ramp to the floor, the spool shifted and the pipe in the center of the spool came off the support stand and struck the employee's right thumb, resulting in a fracture and a laceration.

An employee was dumping trash into a compactor when he lost his balance and fell approximately 5 feet into the trash compactor. The compactor was not in use at the time. The employee was hospitalized with multiple fractures.

An employee was helping to load a customer's truck. The employee stepped toward a forklift to get paperwork from the driver. The employee's right lower leg and foot were run over by the back tire of the forklift. The employee was hospitalized with fractures to his lower right leg.

An employee was descending a 10-foot A-frame ladder. The ladder came out from under the employee when he was about 5 to 6 feet above ground level. He fell, landed on the left side of his body, and sustained fractures to four ribs, his pelvis, and hip socket, and sustained injuries on the left side of his body. The employee was hospitalized.

An employee was unloading a roller from a rollback truck. Another employee unhooked the roller from the winch and the roller slid off the rollback, causing the injured employee to be ejected approximately 1 foot out of the roller onto the ground. The injured employee was on the roller when this happened. The employee suffered a broken right hip. and was hospitalized.

An employee was breaking down boxes to load into a baler. They fell and fractured their right hip on the concrete floor.

Employees were installing wood roof trusses, setting them on concrete masonry unit walls that were capped with new double 2-by-6 top plate. The trusses collapsed in a chain reaction, causing three employees to fall to the slab below. One of them, who had fallen about 20 feet to the concrete, suffered two collapsed lungs, several broken ribs, and a broken pelvis. The employee was hospitalized.

Pile-driving operations were being conducted when a steel I-beam broke loose and fell horizontally. The beam hit an excavator and rolled into an employee who was standing nearby, signaling commands to the excavator operator. The employee's leg was fractured and nearly severed.

An employee was working on a roof. He fell from the roof area onto a concrete pool slab about 30 feet below, striking his head. He suffered a severe laceration to his forehead and a fracture to the right arm.
