Safety Incidents

An employee was working from a temporary plywood platform when a screw loosened, causing him to lose his balance. He fell and suffered a puncture/cut under the right eye (caused by a nail), as well as a broken cheekbone.

An employee was climbing the rungs of a baker scaffold. When he was about 3 feet off the ground, he fell backward from the scaffold. His back struck a short wall as he fell. He was hospitalized, having suffered three broken ribs and a punctured left lung.

An employee was on a mobile scaffold to remove a drywall partition when the partition fell onto the scaffold, causing it to fall. The employee also fell to the ground and sustained a torn hip muscle and a hernia.

An employee was using a baker scaffold to install insulation and drywall in a locker room area when he fell approximately 10 feet to the floor, resulting in contusions to the head.

An employee was working from a 2-foot scaffold. He missed a step and landed on his right leg, rolling his ankle and breaking the leg.

On August 20, 2025, an employee was working from a scaffold while painting the inside of a 30-by-30-foot ground-level water tank. While descending the scaffold, a wheel on the brace broke and the scaffold tipped over. The employee fell approximately 18 feet to the ground and suffered a broken pelvis.

Employees were installing a projector on the ceiling of an auditorium using a scaffold. While the injured employee was getting down from the scaffolding, it became top-heavy and tipped over. The injured employee fell to the floor, landing on his left leg and suffering a broken left fibula and tibia.

An employee was preparing the wing of an aircraft for stripping work and was standing on a four-foot step stand. As the employee was walking on the top platform of the stand, they stepped off the end of the stand and fell to the ground. The employee sustained fractures to their left elbow and left hip.

An employee was on the floor picking up some tools/materials from under a scaffold. A 4x4 piece of plywood was thrown from the scaffold and struck the employee on the head. The employee sustained two broken neck vertebrae.

An employee was on a scaffold caulking and painting a manufactured home. She fell off the scaffold an estimated 6 to 8 feet, landing on the concrete floor. The employee suffered upper body injuries and was hospitalized.

A truck driver was on a platform, pulling a tarp on an asphalt load in a six-wheel dump truck. He fell from the platform to the ground and suffered broken ribs.

An employee was cutting a 4-by-7 concrete slab. The slab fell forward and hit a baker's scaffold, which then fell onto the employee. The employee suffered a broken leg, a broken ankle, and broken ribs.

At about 5:55 p.m. on August 9, 2025, an employee lost his footing while climbing an access ladder to a 12-foot-high spotlight platform. He fell about 9 feet to the ground, breaking eight ribs on the right side.

An employee was on a double scaffold, finishing work on a wall, when the scaffold was moved and fell. The employee jumped off it and landed on dirt, breaking his foot.

An employee was on scaffolding when they fell into a 1-foot gap between the scaffolding and a building. He fell 8-9 feet to the ground, sustaining head injuries and fractures to their arm(s). The employee was hospitalized.

An employee was standing on a scaffold approximately 7 feet high and passing plywood sheets to another employee on a single-story roof. The employee fell backward off the scaffold to the ground below and the sheet of plywood fell on him, resulting in fractures to the left side of his jaw and three ribs.

Employees were dismantling a 20-foot scaffold and shoring system from scissor lifts after the concrete was set when a 4x8 sheet of plywood shoring fell. The plywood struck the injured employee at the ground level below, resulting in fractures to their L1-L4 vertebrae.

An employee was removing stucco from the side of a wall when the ceiling collapsed. The employee jumped out of the way and began descending the side of the scaffold when they fell to the ground. The employee sustained a fractured right wrist.

An employee was walking up the steps of a work platform and missed a step, striking their toe on the edge of the platform. The employee fell to the metal platform and sustained a fractured elbow that required surgery.

An employee was placing masonry blocks on the top of a wall while standing on a scaffold when they fell to the ground. The employee sustained a concussion and bruises.

In the flight line area, an employee was on a work stand platform conducting repairs to an aircraft. While transitioning to another location, he stepped over another employee and lost his balance. The employee reached for the railing to catch themselves, fell, and tore his right bicep. The employee was hospitalized for surgery.

An employee was attaching rigging to a mandrel sleeve during routine mandrel sleeve replacement. The employee fell 15 feet from a work platform to the ground. The employee was hospitalized with fractures to their eye socket, spine, and a rib.

An employee was on a scaffold installing masonry veneer when a plank on the scaffolding fell from its supports, causing the employee to fall approximately 13 feet to the ground below. The employee sustained fractured ribs and a punctured lung.

A painter was painting while standing on the first step of a scaffold. He lost balance and fell 1 to 1.5 feet while striking the scaffold. The employee sustained a laceration and puncture to the abdomen and was hospitalized.

An electrician was on a 9-foot scaffold performing valve maintenance on an 8-inch line near a heat exchanger. The electrician was working to cycle a valve that failed to close. Due to a misaligned drain line and an open manual valve, hot liquid dextrose (~250 F) was released onto the floor below, creating a steam cloud. The electrician was evacuating the scaffold using the scaffold ladder when they slipped and fell 5-6 feet into the hot liquid, sustaining first- and second-degree burns to multiple parts of their body including both arms, both hands, their back, and their buttocks.
