An employee was working to free a load by grabbing and shaking the live side of a tugger line. Once the load became free and progressed upward, the employee's hand was caught and pulled into a snatch block. The employee's right thumb was broken and lacerated. The employee was hospitalized and required a surgical amputation.

An employee was working to repair a machine with a broken metal stopper. She removed the stopper and used a roll of tape to temporarily secure it. While she was moving it back into place to fix the machine, the residual pressure of the plastic pallets forced the stopper down, resulting in her right thumb becoming stuck between two metal parts. The thumb was amputated.

An employee was preparing to move two metal plates (turret arms) using a crane. He placed his left hand on one of the plates to steady it while operating the crane controls with his right hand. As he was maneuvering the crane, the two plates slid together and crushed his left thumb. The thumb was amputated at the interphalangeal joint and he was hospitalized.

An employee was operating a drill rig when when their hand got caught between a 1/4-inch steel cable and a drill rod, resulting in the amputation of four fingers.

An employee was overseeing the operation of a rope being respooled onto the drum of a wire winding machine. He went to remove a piece of wooden reel from the rope. His glove got snagged and his right hand was pulled into the roller, pinching his fingers between the machine's roller and the rope. The middle and index fingers were crushed.

Employees were conducting a snubbing operation. While making up the tubing swivel onto the joint in the mouse hole, employee 2 proceeded to pull power tongs off the pipe. At the same time, the injured employee went to re-adjust the tong heads. The make/brake handle was pulled and the injured employee's right hand got caught in the tongs. The injured employee's index, middle, and ring fingers were amputated and he required surgery.

An employee was working to install a pipe coupling on a section of steel pipe. The pipe became dislodged from its temporary support structure. As the employee went to gain control of the pipe, his right little finger was caught and amputated near the knuckle in a pinch point created by the pipe.

An employee was operating an order picker when his left middle finger was crushed between the lift and storage racking. The employee's fingertip was amputated.

An employee was using a push cart. When he tilted the cart, his finger was caught between it and its handle. The fingertip was amputated.

On September 6, 2025, an employee was moving office furniture. The top of a desk he was moving became dislodged and fell onto his left ring finger, amputating the fingertip.

An employee was operating a hammer drill when his glove got tangled in the drill and his little finger was amputated.

An employee was unloading equipment from a vehicle when items shifted and crushed their left middle finger. The employee initially received stitches and the injury was later classified as an amputation.

On July 29, 2025, an employee was removing a bolt from a metal racking beam. The beam fell, and the employee's hand was crushed between the beam and a pallet of bricks. Two of the employee's fingers were amputated.

An employee was troubleshooting and rodding out a piece of process piping equipment when their left index finger was caught between the tool handle and threaded flange. The employee's fingertip was partially amputated.

An employee was working to offload a trailer when the dock plate fell and amputated his big toe.

An employee was walking in steel-toed boots when his left foot struck a pallet jack, resulting in partial amputation of a toe.

An employee was assisting a forklift operator with dumping trash into a trash compactor when the forklift crushed his right-hand fingers against a guardrail. The employee's little finger was partially amputated.

An employee was assisting the operator of a soil sampling rig to collect soil samples. He went to remove soil sampling tooling and the energized equipment cycled. His right index fingertip was amputated including the fingernail.

An employee was making a delivery and operating the lift gate of his truck when his left little finger was caught in the lift gate. The employee sustained a partial traumatic amputation of the finger through the phalanx.

Employees were drilling a hole when an employee was struck by the drill. Their right leg was amputated at the knee.

An employee was operating a perimeter router. He reached to move the router out of the way so he could advance a panel when he suffered the amputation of the distal phalanx on his left index finger.

An employee was performing maintenance on an a conveyor system. He was working to loosen the chain that moves the belt when his right index fingertip was caught by the chain and amputated.

An employee was driving a forklift with a load on top. As the forklift passed through a door, one of his fingers was caught between the forklift and a concrete wall. He suffered an amputation to the finger.

A truck driver was loading a second roll-off container onto a two-box trailer. He was operating the hydraulic control levers with his left hand, and was holding the D-ring in place on the exterior of the trailer while letting slack out of the winch cable. The employee's right little finger became pinched between the winch cable and the structure of the trailer. The employee required surgical amputation of the distal phalanx.

On April 30, 2025, a field service engineer was working on robots in a cell when the line faulted. The engineer went to unjam the line and the machine caused an amputation to their left index fingertip (about 3/4" total from the fingertip). The machine was not locked out/tagged out at the time.
