Final narrative
On January 6, 2015, at approximately 11:30 p.m., an employee was performing a change over in the secondary turn cell when the clamp/unclamp feature was activated. The employee's fingertips on both hands were caught behind the chuck of the lathe. On the left hand, the third and fourth fingers were amputated.
An employee was aligning/installing the bottom punch in a forging press. An airline was hooked up to extend cylinders for the die pod.
