An employee was running pipe into a hole on a rig floor. He was still running the other joint into the hole when the laydown machine shot the joint of pipe. The pipe hit the employee in the jaw and he fell backward, suffering a fractured cheekbone.

An employee was in the yard area on a forklift pulling lumber. Inclement weather began and the employee stopped the forklift and was walking to a safe location when they were struck by flying debris from the high winds or tornado. The employee sustained a leg fracture.

An employee was working at a job site when something flew into his eye. He was hospitalized with a cut on his cornea.

After reassembly work on a ready-mix truck, an employee turned the truck on to check for leaks. The employee was standing on the truck platform, about 3 feet off the ground, when he heard an air leak. An employee was disassembling a pressurized unit using a driver drill. When the unit was disassembled, it caused a kickback from the tube head, causing the drill to strike the employee's left forearm. They sustained a diaphyseal fracture to their left radius and ulna, which required surgery.

After repairing a piece of industrial equipment, an employee had brake-cleaning liquid on his gloves. A lighter ignited the liquid, and the employee's hands were severely burned.

An employee tripped over a stool and fell on the floor, resulting in a left displaced mid-cervical femoral neck fracture.
