Final narrative
An employee was cutting siding into 2-inch strips using a circular saw on a saw horse. The strip started to buckle; the employee reached to grab it, cutting the palm at the finger joint.
While walking into an office, an employee entered the path of a forklift that was carrying metal siding. The forklift shifted, causing the siding to fall off the forklift and land on the employee's foot. He suffered toe lacerations. Steel-toed boots were worn at the time of the incident.
An employee was unloading an auto clave, stepped off of a concrete auto plate located outside of the work platform, and fell approximately 18 inches into a condensate drain. The drain contained hot water at a temperature of 170 degrees. The employee sustained second degree burns.
An employee was operating a strip saw. As the employee shut off the saw, their right index and middle fingertips were amputated by the blade before it came to a complete stop.
An employee was using a table saw to cut a piece of wood for wall framing. The wood jumped, causing the employee's hand to move toward the saw, amputating their left index finger and causing a laceration on their hand.
An employee was operating a trim saw. While trying to free a jammed board she had just cut, she came into contact with the running saw blade, which amputated her right thumb.
An employee was unloading freight from a truck when a stack of doors fell over and knocked the employee to the ground. The employee sustained a fractured femur.
An employee was performing rebar fabrication using a bending machine. The employee's glove became caught in the area where the rebar bend overlaps onto itself. The subsequent bending/cutting caused the two overlapping portions to squeeze together and amputate approximately 1/4" of his left index fingertip.
An employee was building wood detail blocking and a wood member needed to be resized. The employee took the wood member to the table saw and set the fence to the correct size. The employee pressed down on the board to control it when his left little finger contacted the running blade, resulting in an amputation.
An employee was clearing a pipe along a culvert using a motorized plumbing machine. The end of the machine got tangled and stuck in long grass and other debris. The employee went to lift the end of the machine out of the grass and clear the blockage when his hand was pulled into the machine, resulting in a partial amputation and open fracture of his left distal phalanx.
An employee was stacking concrete blocks for a concrete bunker. As a block was swinging into place, the employee's left leg was pinched between two blocks, resulting in injuries to his left heel and lower leg.
An employee was climbing a ladder to inspect a vent in a restaurant dining area. The ladder slid out on the floor and the employee fell approximately 12 feet onto the ladder and the floor. He was hospitalized with a dislocated shoulder and a back injury.
An employee was filtering a fryer with a fryer filter machine. After going around the corner and then returning to the fryers, the employee stepped into the filter machine. The hot oil burned the employee's right ankle, and the employee was hospitalized.
An employee was cleaning out corn from a grain bin. The sweep auger in the bin was activated and caught his trouser leg. His leg was pulled into the auger, which injured his calf and caused leg fractures and lacerations.
