Final narrative
An employee in the production area was operating 60-ton press machine to form and bend metal parts. The movement of the machine amputated his pointer and middle fingertips on his left hand.
An employee had been moving scrap to a storage area in the fabrication department. While moving the hoist back to the work area, a hook at the end of the hoist got caught on a short rack (about 2 feet tall) of flat steel. The rack was pulled over and struck the employee, knocking him to the concrete floor. The rack then landed on the employee, fracturing both of his legs below the knees.
An employee was using a saw to cut square steel tubing. He pulled a cut tube toward himself to ensure it was cut. Another tube came through and crushed his left ring and little fingers against the side of the machine. Both fingers were medically amputated.
