Final narrative
An employee fell from the upper deck of a drop-deck trailer, landing on the ground on his left side. He suffered arm, rib, and pelvis fractures.
An employee had been moving scrap to a storage area in the fabrication department. While moving the hoist back to the work area, a hook at the end of the hoist got caught on a short rack (about 2 feet tall) of flat steel. The rack was pulled over and struck the employee, knocking him to the concrete floor. The rack then landed on the employee, fracturing both of his legs below the knees.
An employee was using a saw to cut square steel tubing. He pulled a cut tube toward himself to ensure it was cut. Another tube came through and crushed his left ring and little fingers against the side of the machine. Both fingers were medically amputated.
An employee was pulling parts to fulfill a customer order in the parts department. While utilizing a Work-Assist Vehicle (WAV) to pull two 89-lbs. axles, the remaining axles in inventory shifted. One axle rolled forward and struck the employee in the face. The employee sustained a fractured cheek bone.
