An employee was building crates with a pneumatic nail gun. The wood he was nailing split and the nail gun slipped, moving the nail gun to his hand. He shot his hand with the nail gun, requiring hospitalization. The safety pad was in place at the time.

An employee was installing a 15-amp breaker in an electrical panel when a 12-gauge ground wire touched the positive busbar of the panel, resulting in an arc flash. The employee sustained a second-degree burn to their left hand.

An employee was attempting to switch a medium-voltage primary cable and install a 200-amp fuse barrel. The employee contacted the bottom of the switch gear cradle for the fuse barrel, causing an arc blast. The electricity entered the employee s left hand and exited his big toes, resulting in electric shock and burns to the left hand, arm, shoulder, and both feet. The employee was hospitalized.

On December 6, 2023, an employee of Duke Energy was working on a single-phase 120-/240-volt parallel service re-tap when a secondary flash occurred in an underground service. The employee suffered a second-degree burn to the face and was hospitalized.

An employee was at the brake press when the press was activated, causing the ram to move. Two fingertips on his right hand were crushed between the dies and partially amputated.

An employee was loading a piece of round aluminum into a CNC auto-spinning machine. Their left thumb was crushed between the tail block, the aluminum part, and the mandrel. The distal tip of the thumb was amputated.

An employee was seated at a press brake and was putting strips of steel (2-inches by 2-feet) into a brake press machine. The machine actuated and his right ring finger was crushed between the upper die and the steel material. The finger was amputated through the base of the nailbed.

An employee was using a manual spinning machine to cut out the center of an inlet. The cutter bar struck his neck and jaw, breaking the jaw and injuring an artery in his neck.

An employee was climbing a fence in pursuit of two people who had been trying to break into a facility. He fell from the fence, suffered a broken left leg, and was hospitalized.

An employee was moving a tensile strength test device with a dolly. The device fell from the dolly onto the employee's right leg, causing a compound fracture.

An employee was walking past a crossover conveyor when a ramp came down and hit them in the back. The employee sustained a broken back vertebra, as well as a concussion, and was hospitalized.

An employee was using a hook tool to pull a pallet onto the lift gate of a tractor trailer. The hook detached from the underside of the pallet, causing the employee to fall backward off the lift gate. The employee landed on the concrete parking lot about 5 feet below, suffering fractures to the skull and two thoracic vertebrae.

An employee was walking on a truss table when he lost his balance and fell approximately 3 feet to the floor. The employee sustained a dislocated and fractured left ankle.
