Final narrative
An employee was holding a metal diaphragm ring with a nylon stick (tool). The roller used to make the lip on a metal ring (diaphragm) came off of the die inside of the machine, causing the piece to be pulled into the rollers as the machine cycled. The employee's right-hand ring finger was amputated by the rollers.
An employee was sharpening peeler blades when their shirt was caught in the grinding wheel. The employee sustained tendon damage and a crushed right hand.
