Final narrative
On 7/31/2018, at 1:00 AM, an employee was inspecting an auto-pour machine shuttle that was not operating normally. He was bent over with his hand on the rail of the shuttle when the equipment unexpectedly engaged. The shuttle's front roller then amputated his left pinky finger.
An employee was performing maintenance on the injection molding cannister vacuum receiver when the pneumatic cylinder actuated. The employee's finger was caught between the cylinder and the wall of the receiver, resulting in a left index fingertip amputation.
