Federal OSHA recorded a severe workplace injury at Honeywell, 2221 Industrial Rd., NEBRASKA CITY, NEBRASKA 68410 on — Gunshot wounds, affecting the thigh(s).

An employee was on break in his vehicle and had taken his handgun out of his glove box and placed it onto his left thigh/leg. When he was putting the gun back in the glove box, it fired. He was hit in his left thigh, and the bullet exited through his left knee. He was hospitalized.

An employee was operating a stand-up forklift in reverse when his left leg was pinned between the forklift and a guardrail. The employee sustained fractures of the bilateral femoral condyles, lateral tibial plateau and proximal fibula with moderate lipohemarthrosis, a contusion of the medial thigh adjacent to the greater saphenous vein without evidence of significant vascular injury, and a subcutaneous hematoma lateral to the left knee.

A temporary employee was using a stand-up reach truck to put away product in a storage rack area. The truck struck the upright supports on a rack, and the first three fingers on the employee's left hand were crushed between the two.

At about 1:00 p.m. on April 28, 2020, an employee was checking a sulfuric acid railcar to ensure that it was empty before it was sent offsite for cleaning and decommissioning. The employee loosened the railcar's dip tube cap with a pipe wrench, then removed it. Residual pressure in the railcar sprayed about 4 pounds of acid into the employee's face, causing chemical burns to the employee's forehead, cheeks and nose.

An employee was at a firearm range for security training. As she was unloading a firearm inside a vehicle, the weapon discharged. The employee sustained a gunshot wound to the left medial knee.

An employee was test firing a 50-caliber rifle when the bullet ricocheted off a steel plate that was part of the frame for the rubber-coated bullet trap. The bullet struck the employee in the center of his neck, creating a 1/4 inch entry wound. Bullet fragments damaged the employee's trachea and the employee ingested the remaining fragments.

Employees were in a parking lot waiting to board a crew boat. An employee showed his co-workers a pistol and as he was putting it back in his truck, the gun fired. The employee sustained gunshot wounds to the right ring and little fingers.

On 7/31/2018, at 1:00 AM, an employee was inspecting an auto-pour machine shuttle that was not operating normally. He was bent over with his hand on the rail of the shuttle when the equipment unexpectedly engaged. The shuttle's front roller then amputated his left pinky finger.

The roller used to make the lip on a metal ring (diaphragm) came off of the die inside of the machine, causing the piece to be pulled into the rollers as the machine cycled. The employee's right-hand ring finger was amputated by the rollers.

An employee received an amputation injury to his left middle finger while cleaning a water meter that was on a conveyor when his finger was caught in the power-driven roller on the conveyor.

An employee was stacking concrete blocks for a concrete bunker. As a block was swinging into place, the employee's left leg was pinched between two blocks, resulting in injuries to his left heel and lower leg.

An employee was climbing a ladder to inspect a vent in a restaurant dining area. The ladder slid out on the floor and the employee fell approximately 12 feet onto the ladder and the floor. He was hospitalized with a dislocated shoulder and a back injury.

An employee was rappelling down a tree. When he was most of the way down, he fell on the ground and suffered a broken pelvis.

An employee was filtering a fryer with a fryer filter machine. After going around the corner and then returning to the fryers, the employee stepped into the filter machine. The hot oil burned the employee's right ankle, and the employee was hospitalized.
