An employee was pinched between a trailer truck and a 40-yard dumpster after the trailer rolled from where it was parked. He suffered a right thigh laceration and a right shoulder strain.

An employee was measuring a block on a pedal-operated upcut saw equipped with a 10 or 12 inch circular blade. As he was taking a measurement, the blade contacted his left hand, resulting in amputations of three fingers.

Employees were stacking two different types of trusses in a yard. One employee was lifting one end of a truss with a forklift while another inserted a spreader block. The load shifted, and the second employee's left ring finger was pinched between the block and the trusses. The finger was amputated.

The employee was climbing down one of the railcar ladders and slipped and fell between 6 and 8 feet to the ground. On December 13, 2023, an employee was operating a forklift. When they stepped off the forklift, it crushed their left foot between the lift and a racking system. The employee suffered a foot fracture that required hospitalization.

An employee was maneuvering a motorized pallet jack while standing in front of it. He became pinned between the pallet jack and a steel rack cross-beam, resulting in a contusion to his chest/back and a fractured rib.

An employee was stacking empty drums into a trailer. The employee then exited the trailer, using the forks of a forklift as a step. The forklift's parking brake failed, causing the forklift to move forward and pin the employee's right leg between the forklift and the trailer. The employee's right lower leg was crushed, resulting in a fracture and a laceration.

An employee had been spraying weeds behind a school. As he was leaving the field, the employee drove his truck up to the gate, got out, and opened the gate. The truck rolled forward and pinned him against the fence/gate. The gate then gave way and opened from the force of the truck. The employee's left index fingertip got caught in the fence/gate and was amputated.

An employee was using a walk-behind tugger to pull a grinder. The employee's left little finger was pinched between the handle of the tugger and the framework of a staircase. The employee's fingertip was amputated.

An employee was moving a tensile strength test device with a dolly. The device fell from the dolly onto the employee's right leg, causing a compound fracture.

An employee fell while stepping off the tailgate of a pickup truck. The employee landed on the ground and suffered a broken left fibula and tibia.

On October 7, 2025, employees were conducting trenching operations for the installation of a new sewer line system. The excavator's bucket was detached and put inside the trench as it was being exchanged for a different-sized bucket. An employee entered the trench with a shovel to move some dirt located at the front end of the pipe. The excavator bucket shifted and slid, striking the employee s right foot and trapping it against the ground. The employee sustained a fractured right tibia.

WPM-Southern, LLC

Employees were working to dislodge a large truck that was stuck in a rut using tow straps and a front-end loader with the bucket removed. An employee connected the straps to the loader and truck and another employee slowly engaged the loader to close the coupler. The injured employee's arm became caught in a pinch point. The employee was hospitalized with a fractured arm.

An employee was lifting a piece of 30-inch line stopping equipment using a jack screw. A lifting eye weld snapped and the load dropped. The end of the jack screw then landed on the employee's heel and crushed it. The employee was hospitalized.

An employee was helping to lift the grating from a floor draining system when the grating slipped and landed on his hand, resulting in the amputation of his right middle finger at the first joint.

US Battery Manufacturing Company, Inc

A casting machine jammed. An employee's hand was caught in the machine, where a belt line caught and amputated the tip of his finger.

During a workshop meeting in a hotel, an employee heard a drilling noise, so he walked outside to see what it was. An explosion occurred (possible gas line) and his face, ear, and hair were burned. He also fell and sustained a pelvic fracture.

An employee slipped on condensation on a walkway in a parking garage. She fell and suffered a closed fracture to the neck of the left femur.

An employee was using a tool to remove a rag from a roll on the tube mill. The roll pulled the tool and the employee's right hand into the roll, resulting in a partial amputation of the little finger and a fracture to the index finger.
