Final narrative
On or about March 15, 2016, an employee was attempting to remove a bearing from a conveyor when his finger was pinched between a metal gate and the conveyor, partially amputating his left ring fingertip.
An employee was attempting to close an outlet valve due to a power outage. He stepped over an angle iron and fell 12 feet into an open coolant pit filled with 3-4 feet of machine coolant. The employee was hospitalized with lacerations, bruises, a muscle injury, and fatigue.
An employee was driving a tow motor down an aisle. Her tow motor crashed into another one, which was stopped at an intersection, and her left leg was crushed between the two tow motors. Her lower leg was injured and she was hospitalized.
