Federal OSHA recorded a severe workplace injury at BAE Systems, Bldg 211 , WHITE SANDS MISSILE RANGE, NEW MEXICO 88002 on — Soreness, pain, hurt-nonspecified injury, affecting the wrist(s).

An employee was adjusting a halon fire bottle cable when the bottle discharged. His left wrist was exposed to the halon chemical, damaging his arteries and tendons and requiring hospitalization.

An employee was walking when his foot caught residual adhesive from floor tape and he tripped and fell to the concrete floor, resulting in a fractured right hip.

An employee was using a powered pallet jack to move a fixture. His leg became caught between the pallet jack and the fixture and he fell on the same level, resulting in a fractured left fibula.

The injured employee was trying to hold a door open for another employee when the automatic door closed on the injured employee's left ring finger. The employee's fingertip was amputated halfway down the fingernail.

An employee was inside a mid-level ballast tank onboard an amphibious transport dock (naval vessel). He was walking across the deck and stepped onto a board that was covering an open hole. The board split and the employee fell about 15 feet to the bottom of the tank. He sustained internal bleeding, a brain hematoma, and fractured the C6 and L1 vertebrae.

BAE SYSTEMS

While adjusting the rigging for the bull gear installation, the bull gear became unstable and shifted. The shifting gear contacted the mobile platform on which the employee was standing knocking the platform over and causing the employee to fall to the ground. When the gear shifted, the employee's right index finger was caught between the rigging and the wood block resulting in amputation of the finger.

INDUSTRIAL CHEMICALS, INC. (ALABAMA)

On November 27, 2023, an employee was emptying a 2-inch product line that transports sodium hydroxide liquid from a rail car to a 275-gallon tote tank. The nozzle came out of the tote and sprayed sodium hydroxide onto the employee's face. The employee was hospitalized with chemical burns to their face, mouth, and neck.

JOST CHEMICAL COMPANY, INC.

An employee was working with sulfuric acid as part of the production process. While transferring the chemical from a large container to a smaller container, it splashed on his body and hand, resulting in a chemical burn.

Thalle Construction Company Inc

An employee knelt in wet concrete while performing work as a concrete finisher and sustained a chemical burn to the right shin. The employee was hospitalized and required surgery.

Quantix SCS

An employee was using a 5-gallon bucket to unload acid product from a tank. Residual product leaked into the containment area, causing the employee to sustain first- and second-degree burns to the chest, as well as third-degree burns to the arms.

Jones Dairy Farm

An employee was transferring an alkaline cleaning chemical from a bulk container into 1-gallon containers. The employee lifted a gallon container by its label tag. The tag broke causing the container to fall approximately 14-18 inches. The container struck the ground and the contents splashed onto the employee causing chemical burns to their eyes.

Consolidated Wellsite Services

Employees were conducting a snubbing operation. While making up the tubing swivel onto the joint in the mouse hole, employee 2 proceeded to pull power tongs off the pipe. At the same time, the injured employee went to re-adjust the tong heads. The make/brake handle was pulled and the injured employee's right hand got caught in the tongs. The injured employee's index, middle, and ring fingers were amputated and he required surgery.

Geo-Solutions, Inc.

On 9/26/2025, an employee was using a pair of scissors to cut a zip-tie that was securing a box of files. The scissors slipped and lacerated the employee s left wrist. The employee was hospitalized.

CAMIN CARGO CONTROL INC

An employee had been taking fuel samples from the top of a barge. The employee was hospitalized with heat exhaustion.

Celanese Corporation

An employee was troubleshooting and rodding out a piece of process piping equipment when their left index finger was caught between the tool handle and threaded flange. The employee's fingertip was partially amputated.

Cintas Facility Services

An employee was walking in steel-toed boots when his left foot struck a pallet jack, resulting in partial amputation of a toe.

ORSA Technologies

An employee was servicing an exterior mounted environmental control unit while on a 10-foot ladder. He pushed through a pre-made knockout hole on the panel. Upon bringing his arm down, a sharp area along the panel frame lacerated the top of his hand. The employee required surgery.

On 7/31/2025, an employee was training to ride an ATV when it went into a draw and she went over the front of the ATV, landing on the ground. The employee sustained a fractured arm.

An employee was standing behind a frac pump. A discharge hose failed and the employee was burned by hot water on the left upper abdomen. The employee was hospitalized.

US Forest Service

An employee working as a contracted firefighter was using a bulldozer to push back an active fire line when they became entrapped in flames, resulting in first- and second-degree burns.

Hooper Trucking Co

An employee was assisting with the delivery of pipe racks to the drilling rig. The driver was releasing the tie-down lock straps that were holding the pipe racks on the trailer. When the last strap was released, one of the pipe racks rolled off the trailer and struck the employee while they were on the passenger side of the trailer bed. The employee sustained an open wound to the hand and fractures to their left leg and spine.
