A delivery driver was lifting bundles of wire mesh off a flatbed trailer using a truck-mounted lift. As he was lowering the mesh, the lift tipped over and ejected him forwards. He landed on the ground and was injured.

An employee was operating a bar bending machine when his finger was caught between the rebar and mandrel bender wheel, resulting in a right index finger amputation just above the first digit behind the fingernail.

An employee was bending rebar on a fabrication/bending machine when his right pinky finger was caught between the two ends of a piece of rebar as the ends overlapped to form a circle. The employee's fingertip was amputated.

An employee was walking to the front door, slipped on ice and fell to the ground resulting in injury to their left side.

While bending rebar, a piece of rebar sprung loose from the bending machine and struck an employee in the neck. The employee was hospitalized with a torn esophagus.

An employee was tossing a damaged toilet into a compactor. The toilet bounced off the compactor and lacerated the employee's right wrist, cutting an artery and tendon.

An employee was operating a hydraulic jack to bend a frame when the jack came loose and struck the employee. They were hospitalized for a spleen injury.

A truck had been towed into the shop with a flat tire and a blown airbag. An employee checked the tire for damage and was airing the tire. The employee was installing the valve core when the bead slipped off the rim. Air escaped, and the aluminum wheel struck the employee's head and face. The employee became unresponsive and was bleeding.

On December 14, 2023, at 8:40 p.m., three employees were inspecting milling machines for startup. A few of the sleeves on the mill drum needed to be changed out. The injured employee was trying to input a sleeve on the mill drum using a hammer. The hammer hit the edge of the drum, which caused a metal piece to splinter off and strike the employee's cheek just below the left eye. The metal splinter embedded in the back of their eye socket, resulting in pain, swelling, and bleeding that required hospitalization.

An employee was pushing a line of carts into a vestibule when the door cable snapped and struck his right ankle. The employee suffered a fracture that required surgery and hospitalization.

An employee was unloading freight from a truck when a stack of doors fell over and knocked the employee to the ground. The employee sustained a fractured femur.

An employee was performing rebar fabrication using a bending machine. The employee's glove became caught in the area where the rebar bend overlaps onto itself. The subsequent bending/cutting caused the two overlapping portions to squeeze together and amputate approximately 1/4" of his left index fingertip.

An employee was building wood detail blocking and a wood member needed to be resized. The employee took the wood member to the table saw and set the fence to the correct size. The employee pressed down on the board to control it when his left little finger contacted the running blade, resulting in an amputation.

An employee was clearing a pipe along a culvert using a motorized plumbing machine. The end of the machine got tangled and stuck in long grass and other debris. The employee went to lift the end of the machine out of the grass and clear the blockage when his hand was pulled into the machine, resulting in a partial amputation and open fracture of his left distal phalanx.

An employee was operating a router when it cut his right little finger, causing an amputation through the bone.

An employee was helping a coworker transport a tall palm tree with a mini skid steer. The employee was severely shocked by a high-voltage electrical wire above the ground.

An employee was retrieving a Christmas tree from a shelf using a ladder. He missed a step and fell to the concrete floor. He sustained injury to his head and wrist.

An employee was inspecting a generator whose radiator was leaking. He slipped, and a fan blade in the generator amputated his thumb and index finger.

An employee was operating an agricultural tractor during sugarcane harvesting. The employee sustained a lumbar sprain due to vibration or motion from the tractor.

An employee was moving a 3-ton condensing unit, strapped down on a dolly, out of a garage. The strap broke, causing the employee to fall backward onto the brick pavered driveway. The employee suffered injury to a spinal ligament in the neck.
