An employee was unplugging a forklift cord from a wall mounted battery charger unit and sustained and electric shock an a first-degree electrical burn to the left ring finger that resulted in a blister.

An employee was removing mold plates from a pallet when the bottom plate fell forward off the pallet and struck her lower left leg, resulting in a fracture that required hospitalization and surgery.

An employee was trying to dislodge jammed plastic in a plastic sheet line; a shear in the machine amputated the tips of his left middle and ring fingers. The machine was guarded with a metal grate at the time of the incident.

The machine clamped down on his left index finger, degloving the top part of the finger and resulting in a partial amputation.

An employee was performing maintenance on a roll cutting machine in a warehouse. The employee was removing excess grease from the bearings when the machine's chain caught their right index finger. The fingertip was amputated by the chain and sprocket.

During fiberglass work, an employee mixed a methyl ethyl ketone peroxide (MEKP) catalyst with a cobalt chemical that they thought was a dye. A chemical reaction occurred, resulting in an explosion. The employee sustained second-degree burns on his back and a blister on his right arm.

An employee was filtering a fryer with a fryer filter machine. After going around the corner and then returning to the fryers, the employee stepped into the filter machine. The hot oil burned the employee's right ankle, and the employee was hospitalized.

An employee was cleaning out corn from a grain bin. The sweep auger in the bin was activated and caught his trouser leg. His leg was pulled into the auger, which injured his calf and caused leg fractures and lacerations.
