An employee was severely injured and hospitalized after she fell from the top of a load of industrial fans inside the back of a semi-trailer. The fan fell onto the employee. The employee suffered a T12-L2 back injury requiring a two-level fusion.

An employee was changing the drive belts on a fan unit. His left ring finger got caught between one of the drive belts and the motor pulley/sheath as the belt turned on the pulley. His left ring finger was amputated below the first joint and he required surgery.

An employee was assisting with pipe on a drilling rig. The slips came down and landed on the employee's left foot fracturing four metatarsals.

Two employees were struck by the rotary table of a drilling rig while installing caisson. Employee 1 suffered multiple fractures, internal injuries, and head trauma. Employee 2 suffered multiple fractures, internal injuries, spinal injury, and head trauma.

Employees were felling hazard trees. The injured employee was conducting 'bucking' operations to saw a tree from the road when the tree kicked-back, pinning him against the ground and fracturing his left femur. The employee was hospitalized and required surgery.

An employee was greasing tongs on a work-over rig. The tongs lacerated one of the employee's fingers.

An employee was chaining a vehicle when he had to change tools and the compressor. As the compressor was being charged, the employee stepped near the vehicle to make sure the compressor was attached. Then, he instructed a co-worker to lower the compressor. His right hand was on the compressor and under the chain, causing his ring finger to be crushed.

An employee was using a multimeter to ensure proper power supply to a newly repaired rotary screw air compressor before running a voltage test. An electrical arc flash occurred, resulting in burns to both the employee's hands and their face.

An employee was on the roof of a building, putting plastic around a dust collector, when their left middle finger was caught in a grinder insider the dust collector. The employee suffered a partial amputation to the left middle finger.

On August 4, 2025, an employee was fixing a jammed carousel and using a hook to manually adjust the carousel by hand. The hook slipped off and the employee fell to the concrete ground, resulting in hospitalization for a fractured right hip.

An employee was replacing a screen on a rooftop HVAC unit when the rotating exhaust fan blade contacted their right index finger, resulting in a laceration that required hospitalization and the surgical amputation at the first knuckle.

An employee was changing an air filter when the door to the HVAC closed due to wind. The employee sustained an amputation to the left thumb at the nailbed.

An employee was moving a tensile strength test device with a dolly. The device fell from the dolly onto the employee's right leg, causing a compound fracture.

An employee was walking past a crossover conveyor when a ramp came down and hit them in the back. The employee sustained a broken back vertebra, as well as a concussion, and was hospitalized.

An employee was using a hook tool to pull a pallet onto the lift gate of a tractor trailer. The hook detached from the underside of the pallet, causing the employee to fall backward off the lift gate. The employee landed on the concrete parking lot about 5 feet below, suffering fractures to the skull and two thoracic vertebrae.

An employee was walking on a truss table when he lost his balance and fell approximately 3 feet to the floor. The employee sustained a dislocated and fractured left ankle.
